Family Care
Family Care is a program in Wisconsin that offers eligible seniors with a wide variety of long-term care services, making it possible to remain at home and connected to the community.
As a Family Care member, you will have support from a Care Management Team comprised of a care manager and nurse. Once enrolled, your team will conduct a comprehensive assessment of your needs, abilities, preferences and values. Together, you will look at:- What you are able to do for yourself
- Supports you have in your life already
- Needs you currently have
- What is important to you
- Your outcomes and goals
You and your team will then develop a personalized plan to meet these outcomes and goals.
With this program, you will continue to choose your own physicians while Family Care provides you with the added long-term care support you need. Your Care Management Team (the care manager and the nurse) may make suggestions and may assist in some coordination, but you and your doctor will continue to be responsible for managing your health and medical care decisions. You will continue to use your Forward/MA card to obtain needed medical care.

The list above contains only a small fraction of the benefits available through Family Care to keep you independent as long as possible. You and your team work together on managing your long term care needs and services, with some attention to maintaining health and preventing health problems. This program provides additional independence since managing your health and medical care still remains with you.
Family Care is a good fit for people who:- Desire better choices about the services and supports they need to meet their needs
- Like having a personalized plan of supports designed around their needs and preferences
- Like the option of self-directing their own supports and services
- May want to work and gain knowledge from benefits counseling in order to make informed decisions regarding future vocational plans
- Are looking for creative solutions that focus on the whole person
- Have cognitive or behavioral challenges that need specialized care plans
- Want one place to go to where they can learn about community resources and government programs as well as receive clear information about all of their options
- Want to stay as independent as possible for as long as possible
